Community Navigation

Diverse Community Navigators will connect individuals and families to needed services and will:

• Identify and collaborate with new organizations and services.

• Strategically focus on addressing social determinants of health.

• Complete individual and family need assessments and navigation plans.

• Collaborate and communicate with intensive case managers 

• Provide individual and group education as needed.
