Our Objectives...

Identify and collaborate with local organizations to identify and serve our target populations.

Provide Intensive Case Management and Navigation Services targeting at least 150 new individuals and 80 - 100 families annually.

Provide Intensive Clinical Case Management Services targeting at least 50-60 pregnant women annually.

Strategically collaborate with community organizations, churches, food banks, education, housing, behavioral health, healthcare and other organizations to meet the needs of individuals and families.

Provide technical assistance, training and consulting services to organizations serving high-risk populations. Services will include but are not limited to program planning, health disparities, social determinants, cultural competency, communications and quality improvement.
